Paranoia is a rune effect in Mass Confusion, a Spirit Realm Witch Doctor skill.

This rune effect functions as a damage buff, causing any enemy in the area of the Mass Confusion to take 20% additional damage for the duration of the effect.

This effect turns the skill into a double bonus, as all the monsters in the area are tagged with the bonus damage, whether they are confused or not. Since Confused enemies are much easier to hit and kill, this is a way for the Witch Doctor to make enemies stand still, not fight back, and take extra damage for a whopping 12 seconds, which is vastly longer than any other stun or confuse effects in the game.
